Traci Diveley, PTA
Field Operations Director

Traci serves as Field Operations Director where she has oversight of scheduling and supervision for physical therapists/assistants, occupational therapists/assistants, and speech language pathologists that provide rehab services for area nursing home facilities and home health agencies. Traci graduated from Illinois Central College in 1986, joining Azer Rehab Systems, P.C. in 1992. Her clinical experience includes long-term care, home health, outpatient, and sports medicine.
