Dawn CarlsonPhysical Therapist Assistant
Dawn graduated with an Associates Degree of Applied Sciences from the Physical Therapist Assistant Program at Black Hawk College in Moline, IL. Dawn gained experience from her PTA Clinicals through out-patient facilities and nursing home settings. She brings her experience to the geriatric community in nursing home facilities. She provides physical therapy in Azer Clinic's contracted area longterm care facilities and home health services. Dawn has been with Azer Clinic since August, 2006.
